How often you actually need a clean

The “every six months” rule is a sensible baseline but not a one-size-fits-all answer. Recall frequency depends on three factors:

Low-risk adults (most patients) — every 6 months

You brush twice daily, floss most days, no history of gum disease, no active decay, no orthodontic work, non-smoker, healthy diet. A 6-monthly check-and-clean catches problems early.

Higher-risk adults — every 3–4 months

• History of periodontitis (gum disease)
• Smokers — smoking suppresses immune response in the gums
• Diabetics — particularly poorly-controlled type 2
• Pregnant women — hormonal changes increase gingivitis risk
• Active orthodontic treatment (braces or Invisalign)
• Patients with implants — peri-implantitis is harder to spot than gum disease around natural teeth
• Dry mouth from medications, Sjögren’s, or radiation therapy

Children — every 6 months from age 1–2

The Australian Dental Association recommends a child’s first visit by the time the first tooth erupts (usually 6–12 months) — this is a “lap-and-look” with the child sitting on the parent’s lap. From age 2 onward, regular 6-monthly visits including age-appropriate cleaning, fluoride varnish, and fissure sealant assessment.

Exactly what happens during a clean

1. Examination (5 min): the dentist or hygienist looks at every tooth, checks fillings, looks for decay, and inspects gums. Any concerns get noted in the record.
2. Periodontal probing (3–5 min): a fine probe measures the gum-pocket depth around each tooth (1–3mm is healthy; 4mm+ suggests gum disease starting). This is the single most important diagnostic test for gum health.
3. Ultrasonic scaling (10–15 min): a small vibrating tip with water spray gently removes hardened tartar (calculus) from above and below the gum line. The vibration breaks the tartar bond without damaging the tooth.
4. Hand scaling (5–10 min): fine instruments (curettes and scalers) catch any deposits the ultrasonic missed, particularly between teeth and at the gum line.
5. Polishing (5 min): a rotating rubber cup with prophylaxis paste removes surface stains and gives the smooth feel patients associate with a “good clean”.
6. Flossing demonstration (3 min): the hygienist shows you any spots you’re missing and demonstrates correct technique. Often the most useful part of the visit.
7. Fluoride varnish (optional, 2 min): a sticky tooth-coloured varnish painted on with a brush. Strengthens enamel for 24–48 hours and is particularly recommended for decay-prone patients.
8. Findings discussion (3–5 min): the dentist summarises what was found, books the next recall, and arranges any follow-up treatment.
